Sending Forms to Clients

Use the Send Form feature for all forms that the client must fill out on their own (I.e. Release of Patient Information or a pre-session GAD-7)

  1. Navigate to your Care Dashboard.
  2. Click the Clients tab on the left side.
  3. Select the relevant client.
  4. Click on any appointment in the client’s session list.
  5. Once on the Client Chart, click the Forms tab at the top of the page.
  6. Click the blue Send Form button in the top right corner.
  7. Select the relevant form from the dropdown.
  8. Click the blue Send Form button to confirm.
