# Referring a Client Outside of Emora

1. Go to your [**Care Dashboard**](http://www.emorahealth.com/care). 
2. Open the **Client** Tab.
3. Click on the Client.
4. Under the client's name click **Transition Care**
5. Select **Refer**
6. Select the type of care the client needs, including IOP, PHP, In-Person, or Eating Disorder Treatment.
7. Select from the list of vetted partners or suggest a new partner to send client information 
     1. If suggesting a new partner, fill out relevant information on the organization and click **Add Partner**
8. Add any Clinical Notes as needed
9. Click **Review** 
10. Select what happens to your caseload after the referral: 
      1. **Transfer care** - client will be discharged from your caseload and continue with the referred provider
      2. **Continue co-care** - continue seeing the client while they also receive care from the referred provider
11. Select whether you consent to sharing clinical information with the referring provider
12. Confirm the referral and click **Submit Referral **
