# Completing Care for a Client

1. Go to your [**Care Dashboard**](http://www.emorahealth.com/care). 
2. Open the **Client** Tab.
3. Click on the Client.
4. Under the client's name click **Transition Care**
5. Select **Discharge**
6. Fill out required answers to capture treatment outcomes, including whether they met their treatment goals, areas of improvement, and overall progress
7. Click **Review**
8. Review completed care summary and click the acknowledgement to confirm.
9. Click **Complete Care** at the bottom right corner.

Note: Use this path only for clients who met their treatment goals and are graduating from care.
