# Completing a Biopsychosocial Assessment

 

Completing a Biopsychosocial is not required for intakes, but we do have one for adults available if you prefer to use it. 

1. Go to the **Clients** tab on your Care Dashboard.
2. Find or search for the specific client and click **Client Chart**
3. Click the **Notes** tab at the top of the page.
4. Click **Create note** in the top right corner
5. Select **Form / other note**
6. Select the **Biopsychosocial Assessment (Adult) **
7. Click **Create note**
8. Fill out the form and click **Save and Sign**
