Completing a Biopsychosocial Assessment

 

Completing a Biopsychosocial is not required for intakes, but we do have one for adults available if you prefer to use it. 

  1. Go to the Clients tab on your Care Dashboard.
  2. Find or search for the specific client and click Client Chart
  3. Click the Notes tab at the top of the page.
  4. Click Create note in the top right corner
  5. Select Form / other note
  6. Select the Biopsychosocial Assessment (Adult) 
  7. Click Create note
  8. Fill out the form and click Save and Sign
