# Creating Other Forms

*Use the **Create Form **feature for all forms that require your input before sending (i.e. Safety Plan, Excuse or Accommodation, or an in-session PHQ-9)*

1. Navigate to your **[Care Dashboard](http://www.emorahealth.com/care)**.
2. Click the **Clients** tab on the left side.
3. Select the relevant client.
4. Click on any appointment in the client’s session list.
5. Once on the **Client Chart**, click the **Forms** tab at the top of the page.
6. Click the white **Create Form** button in the top right corner.
7. Select the relevant form from the dropdown.
8. Click the blue **Next** button to continue.
9. Fill out each relevant section.
10. Once complete, click the blue **Save and Send** button to share it to your client's dashboard.
