Creating Other Forms

Use the Create Form feature for all forms that require your input before sending (i.e. Safety Plan, Excuse or Accommodation, or an in-session PHQ-9)
  1. Navigate to your Care Dashboard.
  2. Click the Clients tab on the left side.
  3. Select the relevant client.
  4. Click on any appointment in the client’s session list.
  5. Once on the Client Chart, click the Forms tab at the top of the page.
  6. Click the white Create Form button in the top right corner.
  7. Select the relevant form from the dropdown.
  8. Click the blue Next button to continue.
  9. Fill out each relevant section.
  10. Once complete, click the blue Save and Send button to share it to your client's dashboard.
