Creating Other Forms
Use the Create Form feature for all forms that require your input before sending (i.e. Safety Plan, Excuse or Accommodation, or an in-session PHQ-9)
- Navigate to your Care Dashboard.
- Click the Clients tab on the left side.
- Select the relevant client.
- Click on any appointment in the client’s session list.
- Once on the Client Chart, click the Forms tab at the top of the page.
- Click the white Create Form button in the top right corner.
- Select the relevant form from the dropdown.
- Click the blue Next button to continue.
- Fill out each relevant section.
- Once complete, click the blue Save and Send button to share it to your client's dashboard.
